ZIP codes come in only 1 form: 12345
ZIP+4 is: 12345-6789
But most users won't use the ZIP+4 form, yet you should allow for it.
Consider
it's use optional. I know my +4 code, but I'd bet that a high 9x% of people
don't know theirs.
The use of "CA" in front of the ZIP is never proper. Maybe your form
doesn't
have a field for "State" or "Province"?
