I did actually find a website after doing some searching on google and found
this site. http://opensrs.iwpp.com/
It's a little out of date, but I was able to use this as an example to get
our site up and running with the customizations we needed. You'll notice
that some of the fields won't match up, but overall it's a good guide.

We've got it up and running at http://register.delaware.net
