I just applied a minor patch to the manage.cgi. Look for the appropriate input field. Its just a matter of changing an HTML tag.

Look at our site on http://domains.sime.com/agreement.html and http://domains.sime.com/udp.html . I think this is what they want.
Please note that you have to fill out some of the blanks in the agreement (your company name, ...)
