Yes, it is very common. It is one of the main sources of frustration in
this business. It is widely suspected that NSI does it on purpose, but of
course, there's the old saw about not attributing to malice what can be
explained by incompetence alone.

Ross at OpenSRS has put in a lot of effort coming up with a standard for
transfers between registrars which will hopefully begin to make a
difference if/when it is put into practice. But for now, there is nothing
you can do about it except try again when they fail (and try the
transfers early, so that a failure does not lead to the domain name
expiring).
