HTML table? I suppose -- the link is to an entry in "The Farm" (the
Tucows developer blog). I also posted the questions in plain text to
the "developer" and "discussion" mailing lists.

My job is to win developer "hearts and minds" and I've been great
latitude to do so. Therefore, if a web form will get the information, a
web form shall be made. I'll whip one up and post the URL as soon as
it's up. Expect it early in the week.
All I ask is that those who insisted on a web form visit it and fill it
out. Sound fair?
