Transfer request via FAX

Hi,
a customer asked us to transfer his domain, but the email address of the
admin-c is unreacheable. If I remember correctly, there is a way to ask for
transfer using fax but I don't know exactly where download the template,
number and the procedure at all.
Any hints ?
